PLUS offers
a full continuum of residential services ranging from supervised group
homes to independent apartments. Individuals are admitted to and move
through residential program levels based upon assessment of their needs
and current levels of cognitive, behavioral, physical and psychosocial
functioning. Individuals move from one residential level to another based
upon the kinds of supervision and/or support they require in the following
functional activities:
Self-care
Personal hygiene
Money management
Self-medication
Health care
Nutrition
Stress management and behavioral self-control
Transportation and accessing community (vocational, education,
& recreational) resources
SUPERVISED
LEVEL
Persons in
the Supervised Living Program require intensive, highly individualized
staff supervision, training, and support.
SUPPORTED
LIVING
Individuals
in the Supported Living Program do not require continuous staff supervision,
but do need highly individualized support and training in one or more
functional activities.
INDEPENDENT
LIVING
Persons in
the Independent Living Program are able to independently complete all
functional activities, but require highly individualized coaching or trouble-shooting
from their Community Facilitator in order to maintain independence.
