PLUS
offers a full range of long-term services for persons with acquired
brain injuries who no longer require active daily neurorehabilitation.
However, these individuals do need ongoing support, supervision,
maintenance of functional skills, and program activities that enhance
the quality of their lives.
Long-term
services are highly individualized to meet the unique needs of each
person. Services are available to both residents and outpatients.
Long-term services are provided at each PLUS Day Program, in various
PLUS residences, and in the community.
Each
person receiving long-term services is assigned a Case Manager who
serves as their advocate, neurorehabilitation and health care coordinator,
as well as liaison with family and payers. The treatment team regularly
reviews the changing needs of these individuals with them and their
families at the Individual Rehabilitation Planning meeting. Available
