The Independent
Living Program at Maple Shade
While a limited
number of independent residential living slots are available as part of
the continuum of care at the Lawrenceville and Galloway locations, the
Independent Living Program at Maple Shade, established in late 2000, is
devoted entirely to independent residential living.
Admission
Criteria
Admission
to Maple Shade is available to qualified present PLUS residents, as well
as to qualified new admissions. Acceptance to Maple Shade requires that
the individual be capable of independent self-care and personal hygiene,
money management, self-medication, physical and mental health care self-management,
as well as independent transportation and access to community resources.
Rehabilitation
Program
Community
Facilitators provide daily individual Cognitive Rehabilitation aimed at
fine tuning independent life skills in real-life, real-time situations.
Residents also participate in weekly case management and individual and
group counseling. All participants are required to pursue volunteer or
paid part-time community employment and/or educational opportunities at
local adult schools or colleges. Participants may also choose to participate
in the Creative Industrial Arts Program at Lawrenceville for several days
each week.
Discharge
Options
For some
participants successful completion of the Independent Living Program at
Maple Shade is the final step before return to complete independence in
the community. These individuals may also choose to participate in follow
up or transitional resources from PLUS Outpatient Programs in Lawrenceville
or Galloway. For others, Maple Shade may provide long-term placement in
the least restrictive setting. Over time, as increased support becomes
necessary for some participants, return to greater levels of support and
supervision within PLUSs continuum of care is also available.
