PLUS Day
Programs provide a comprehensive continuum of care. Individuals advance
through Day Program levels based upon treatment team review of relevant
history and current levels of cognitive behavioral and psychosocial functioning.
Within each program level, participants are offered a combination of individual
and group therapy, as well as retraining opportunities specifically designed
to meet their personal rehabilitation needs.
Community
Readiness
Individuals
just discharged from acute rehabilitation hospitals after recently sustained
brain injuries, and those who have been discharged from long-term inpatient
programs, typically enter PLUS Day Programming at this level. The Community
Readiness program emphasizes the reacquisition of cognitive, behavioral,
social, and physical skills through extensive individual treatment sessions
and small group activities. Individuals at this level may also begin the
process of basic academic and prevocational retraining. For persons at
the Community Readiness level, most of the treatment is spent at the PLUS
Day Program.
Community
Re-entry
Individuals
at the Community Re-entry level have either demonstrated sufficient basic
cognitive, behavioral, social, and physical skills, or have developed
sufficient compensatory strategies in these areas, to begin the process
of practicing these skills in real-life, real-time settings. For example:
a person who has mastered basic math skills in the Community Readiness
Program would put these skills to practical application in the Community
Re-entry level by learning budgeting and banking skills. Likewise, a person
who has mastered prevocational skills in the Community Readiness Program
would begin the process of obtaining volunteer or paid supported employment
in the Community Re-entry Program. The treatment day for people in this
program is divided between individual and group therapeutic activities
at a PLUS Day Program location and community based vocational, educational,
and recreational activities.
Community
Activities Program (CAP)
Individuals
in CAP respond best to a schedule of high-interest, low-stimulus, hands-on
activities with minimal transitions. Day Program based groups often last
for several hours and are devoted to activities such as cooking, crafts,
creative industrial arts, and cognitive games. Individual treatment sessions
are geared either toward the acquisition of new cognitive, behavioral,
social, and physical skills, or toward the maintenance of previously attained
gains in these areas. Several times each week these individuals participate
in community based recreational activities such as bowling and community
service activities such as volunteering at the local food bank.
Independent
Program
Individuals
residing in their own homes or in a PLUS Independent Living Program are
often involved in busy schedules of community based vocational, educational,
and social activities. These people no longer require an organized Day
Program, although they may continue to benefit from individual and group
sessions specific to their present rehabilitative needs. PLUS treatment
teams work with these individuals to develop highly individualized schedules
of therapy "appointments" in a PLUS Day Program that fits into
their busy and productive lives. Persons at the Independent Level are
expected to arrange their own transportation to and from their PLUS appointments,
just as they would for other community based activities.
