PLUS offers home and community based services for persons with acquired brain injuries who live in their own residences or with their families in communities nearby PLUS Program sites.

These services are highly individualized and tailored to the unique needs of persons who are in the final stages of community reintegration. Persons eligible for these services include those who have graduated from PLUS Residential Programs, PLUS outpatients, and individuals who have not previously participated in PLUS Programs.

Home and community based services are provided by Community Facilitators who are trained Cognitive Therapists. Services are rendered in the individual’s home and its surrounding community. The goal of PLUS’s home and community based service is to support individuals in achieving full independence by practicing the skills they have learned in formal therapy session in real-life, real-time situations in their own homes and their own community.

Areas addressed by the Community Facilitator include:

• Medication Management
• Banking
• Public transportation
• Health care
• Leisure activities
• Employment
• Housekeeping
• Shopping
• Brain injury support services
• Educational opportunities

The end result of PLUS home and community based services is a person who is independently able to manage his or her home and access all of the resources they require to maintain a high quality of life in the community of their choice.
